The blackface bug occurs when the meshes and textures for the NPC's face are either missing or don't match the data in game records. This is usually because a mod modifies the facegen data of an NPC record without also exporting new meshes and textures for the face that matches that new data, or if it's a new NPC, because the mod author never exported the facial meshes and textures to begin with.
